We pride ourselves in developing a personalized care plan for our clients and strive to ensure that we are taking care of our clients' needs, so they can maintain their independence, dignity and remain in their current, preferred living environment.
What to Expect
Step 1
A Care Manager conducts a home assessment to help determine what you need from us to remain safely in your home.
Step 2
We develop a care plan tailored to your needs.
Step 3
Client intake forms are completed and a visit from one of our Home Health Aides is scheduled.
Scheduling, Billing, & Payments
Home Health Aide visits are scheduled out a month at a time by our Executive Assistant or our Home Health Aide Lead.
You will be asked to share in the cost of home care services based on your monthly income and the type of services being performed. North Shore Area Partners will send you a monthly invoice for services rendered the prior month.
We accept payment via check or credit card (5% processing fee added). If you or your loved one has Long Term Care Insurance, our staff will be happy to assist you in navigating the insurance process. Additionally, we are licensed under the Elderly Waiver—if you’re not sure if you qualify, please contact our Home Care Manager at 218-353-1464.
